Counselling referral form

Counselling referral form to be completed by a legal guardian or parent of student. Please provide as much detail as possible. All referrals will be prioritised according to the information provided. Once completed acknowledgement of completion screen will appear. The college counsellor with endeavour to make contact within 2 business days to discuss the referral and processing thereof. If the student has expressed thoughts of suicide, self-harm or harm to others please contact the family GP immediately. DO NOT leave the student unattended

Counselling consent form

The Counselling consent form to be completed by parent or legal guardian of a student prior to the commencement of counselling sessions with the College Counsellor. No sessions will be undertaken without consent having been provided.
